Mentor Program Caseworker (Bilingual English/Spanish)

Location: Hybrid Remote/In-Office
Salary Range:   $43,000 - $45,000 per year

Position Summary: The Mentor Program Caseworker position focuses on providing a support system to prospective teen mentees to stabilize the social determinants that would hinder the successful completion of high school or a GED program. The Caseworker assesses the needs and connects them with relevant community organizations and social services, inclusive of health care navigation, parenting support and basic needs. In collaboration with the Program Coordinator, the Program Caseworker is also responsible for community networking to recruit volunteers to join the program as mentors and encourage ongoing teen mom referrals.  Services may occur in the office, at school, at home visits or virtually, based on client need. Local travel required.

Essential Functions and Responsibilities:

  • Assess teen client’s concerns and needs on social, emotional, economic and environmental factors.  Utilizing a client-centered approach, create an individualized short and long-term goal oriented case plan.
  • Provide case management and link client to other HMHB and community programs and services. Including enrollment into a high school education program and college prep programs.
  • Provide support to mentors and mentees when navigating situations or challenges that require social services referrals and or casework.
  • Facilitate recurrent on-site mentor trainings and execute program related activities such as mentor/mentee events, including end of year graduate celebration.
  • Facilitate teen peer support groups with the collaboration of the HMHB Maternal Wellness Coordinators (Circle of Moms).
  • Serve as a program liaison with high schools and community organizations to maintain collaborations and increase recruitment into the program. Attend in-person meetings to promote the program to potential partners.
  • In partnership with the Program Coordinator, follow up with mentor inquiries by conducting inperson interviews to assess eligibility and aptitudes for mentoring.
  • Attends pertinent meetings, trainings and conferences related to the position and as deemed by Supervisor.
  • Work with Program Coordinator, Senior Program Director, Chief Program Officer and Chief Executive Officer to provide data reports for grant reports in a timely manner.

Knowledge, Skills and Abilities:
  • Excellent interpersonal, relational, problem solving and communication skills; verbal and written.
  • Considerable knowledge of principles and practices of working with diverse populations.
  • Computer skills including Microsoft Office Suite.
  • Ability to manage multiple priorities and manage time effectively to meet established timelines for work completion.
  • Ability to be flexible in regards to work location and work schedule including periodic work on weekends and weeknights.
  • Bilingual preferred (English/Spanish).
Experience Required:
  • Experience working with youth in a social service setting.
  • Experience with case management and social services.
  • Experience working with volunteers. Educational

Requirements:
  • Bachelor’s degree in psychology, social work or social sciences related field or equivalent combination of education and experience.
